- Doctor’s visit: Consult a doctor of your choice to obtain a First Certificate of Capacity, which is required to proceed with your claim.
- Claim Form: Complete a Workers’ Compensation Claim Form, which is available online. If you encounter difficulties, seek assistance from your employer or contact official advice lines.
- Submit documents to your employer: After completing the First Certificate of Capacity and the Workers’ Compensation Claim Form, make a copy for your own records and give the original documents to your employer.
- Employer document submission: Your employer is required to lodge your claim with their insurer within 5 working days.
- Insurer notification: The insurer will assess your claim and notify you of the outcome within 14 days of receiving your claim form.
