Comprehensive, bulk billed health assessments

Designed to identify health risks and promote preventive care, our health assessments are conducted by our skilled nurses and followed up with detailed GP consultations. These assessments help us make informed recommendations tailored to your specific health needs. Best of all, they’re fully bulk billed, reflecting our commitment to accessible healthcare for everyone.

Please review the following list to see if you qualify for one of our health assessments:

  • 45 to 49-year-old health check: Available, once, for those in this age group at risk of developing chronic disease.
  • Over 75 health check: An annual assessment for seniors over 75. This is a great opportunity to have a comprehensive health check as part of your yearly drivers licence medical, if you’re 80+ or have any medical conditions that make affect your ability to drive.
  • Comprehensive medical assessment: Annually for residents in aged care facilities.
  • Type 2 Diabetes risk evaluation: Every 3 years, for individuals aged 40-49 years old, assessed to be at high risk of developing type 2 diabetes.
  • Health assessment for intellectual disabilities: Can be offered annually.
  • Refugee health assessment: Provided once for refugees or humanitarian entrants.
  • Aboriginal and Torres Strait Islander health check: Offered every 9 months for all age groups.

No, all our health assessments are bulk billed, meaning there are no out-of-pocket expenses for eligible patients under Medicare.

What should I bring to my health assessment appointment?

Please bring any relevant medical records, a list of current medications, and information about your medical history.

After hours support

For medical emergencies, dial 000. Plantagenet Hospital’s Emergency Department is open 24/7.

We offer after-hour appointments some Monday evenings, billed at the full private rate. Book online via Automed or call our admin team: (08) 9892 1000. For free advice after hours, contact Health Direct at 1800 022 222.

If you’re a regular patient in need of after-hours consultation, you can call (08) 9892 1000. Follow the prompts towards the end of the voice message, and you can be put through to our on-call GP for advice. Please note… There is a $150 fee for an after-hours phone consultation of up to 15 minutes duration, payable on the next business day.There may not be any Medicare rebate available, depending on your specific situation. If you require an after-hours home visit, your doctor will advise you of the private rate prior to attending.
